I have consented into the study but haven’t received a questionnaire?
Thank you very much for consenting into the study, the questionnaires are sent out in bulk at regular intervals to our new recruits. If you still haven’t received one in five working days please do let us know so we can follow up for you.

I have filled in the questionnaire, when do you want me to come in for a bleed?
Thank you very much for taking part in our study. We will be contacting those participants chosen for an additional bleed over the coming months and you will be contacted directly with the details you have provided us with to arrange an appointment should we need additional samples from you.

I don’t know my Site ID or Participant Number
